ABLE Procedures

Recommended ABLE Loan Procedures &
Check-In / Check-Out for Students and VR Clients

  1. An IEP or IPE Team Meeting could amend the student's (or client's) present IEP (or IPE) to reflect the ABLE device loan for the student to try out in a certain time period.
  2. A copy of the student's or client's IEP or IPE, including release/evaluation information is required.
  3. This IEP or IPE evaluation should be documented in a written report by school personnel and/or therapist with the student or client in using the device at home, school and/or community environments according to his/her progress with the IEP or IPE goal(s) and objective(s).
  4. A Loan Agreement Form will be provided to the requesting person(s) by the NMTAP ABLE Device Loan Program Manager. The form must be signed by the requesting parties and the ABLE Device Loan Program Manager. After receiving the signed form, the assistive device will be mailed out.
  5. During the duration of the Loan Agreement, the NMTAP Device Loan Program Manager may provide (or assign others to provide) technical assistance and training on the assistive device.
  6. NMTAP staff will provide follow-up services midway(delete word) through the loan period to get feedback from the recipient.
  7. One week before the Loan Agreement expires; NMTAP will provide "return of equipment" instructions. *NOTE* A report of the condition and utilization of the device is required from the therapist and/or teacher and must accompany the equipment upon its return to NMTAP.
  8. The Loan Agreement will be recorded as "returned" after ABLE Device Loan Program Manager has inspected the device(s) for damage or missing parts and received the report on the utilization of the device.
